"WHAT IS TELEHEALTH??? Well to put it simply, it's an amazing new resource that allows your student to be seen virtually, by a licensed healthcare provider while at school. Linked below is the information and required forms that need to be filled out in order to participate in telehealth this year. The first page contains a link to complete the forms online or on a tablet or smartphone. Be sure to complete all the necessary forms to be eligible for telehealth services. The forms may also be printed and returned to Nurse Jaci at school. Feel free to reach out if you have any questions or concerns."
